Goods and Services Tax - Canadian Companies/Organizations Companies/organizations located in Canada are required to pay Goods and Services Tax (GST) on all Project Scorecard™ services. Please select one category below that best identifies your Company/Organization:

If your Canadian Organization has GST exempt status, you must provide Allan Lowe & Associates Inc. with proof of your GST exemption. If you have checked this status, Allan Lowe & Associates Inc. will contact you to arrange for this documentation. Payment Process* After you submit this Detailed Information Package Order Form, you will be invoiced by Allan Lowe & Associates Inc. for $200 CAD for the Detailed Information Package. Upon receipt of payment, you will be forwarded the Detailed Information Package. The package provides easy to use step-by-step guidelines to create the Project Evaluation Statements and identify participants for your Project Scorecard™. Payment Options For your convenience, you have 4 payment options for your Detailed Information Package and future Project Scorecards™. Please select your preferred payment option:

	Option 1 - Major credit cards – securely processed through PayPal (no PayPal account required)
	Option 2 - PayPal – (PayPal account required)
	Option 3 - Direct online banking payment
	Option 4 - Invoice from Allan Lowe & Associates Inc.

	Note: Payment for the Detailed Information Package is required prior to Allan Lowe & Associates Inc forwarding the package to you. Using Payment Option 4 will delay your receiving the Detailed Information Package, especially on international orders

Your $200 payment is non-refundable. However, it will be applied towards the one-time setup fee for your initial Project Scorecard™.
